Current research · Federal reporting readiness

When Federal Reporting Becomes an Organizational Design Problem

In August 2026, the U.S. Department of Education reported that more than 1,900 institutions had not reported or had under-reported required Financial Value Transparency and Gainful Employment (FVT/GE) data for the 2024 and 2025 cycles, and set a final deadline of January 15, 2027 for the overdue data. By late September 2026, about 1,550 institutions were still short.

THE THREAD is studying why. Through current advisory work with a private university of fewer than 1,000 students, we are examining what it takes for a small institution with a lean team to move from deadline-driven reporting to a continuous institutional intelligence system: data collected once, governed, integrated, and reused for compliance and decisions. Our working hypothesis is that the gap is rarely technical alone. It is organizational: ownership, workflow, data governance, and capability.

Dr. Leonie H. Mattison

Founder & Principal

I’m Dr. Leonie H. Mattison, founder of THE THREAD, an organizational and talent development firm that helps boards, chief executives, and founders navigate growth, leadership transition, organizational performance, and responsible AI adoption by aligning people, systems, strategy, governance, and execution.

For more than 20 years, I have led at the president, CEO, and COO levels across higher education, government, public health, and human services. The challenges my clients bring to THE THREAD, from developing talent and redesigning systems to growing revenue, strengthening performance, and leading through transition, are ones I have carried myself.

Across sectors, I have done this work at scale. During the Great Recession, I directed regional workforce innovation and economic development, moving 942 residents through training and 212 into jobs. I also grew a public health grant portfolio from $20 million to $65 million; built organizational and talent development systems serving 4,600 employees across 23 county departments; modernized a multi-site health and human services organization through the COVID-19 pandemic, keeping services running across more than 20 locations; and guided a post-founder institutional transition that produced 40% growth in new enrollment and 54% growth in net assets within two years. Over my career, I have secured and stewarded more than $300 million on behalf of institutions, people, and communities.

A university educator and author of 13 books, I was named a 2023 Top Women in Business editor’s pick for higher education by the Pacific Coast Business Times and contribute to the Forbes Business Council. That experience shaped the T.H.R.E.A.D. System™ and the Institutional Coherence Index, the proprietary approaches my firm uses to surface the patterns beneath results and help leaders decide what to strengthen, change, or redesign. Is this a reorganization?

Why THE THREAD

From the president’s office, not the sidelines.

Dr. Leonie H. Mattison served as President and CEO of Pacifica Graduate Institute through a post-founder identity transition and a post-pandemic enrollment decline, leading the institution to a six-year WSCUC reaffirmation of accreditation in 2025.

She has served as faculty at Antioch University, where she taught Strategic Management, and has lectured at Santa Barbara City College, Westmont College, Saybrook University, and Middlesex County College.

Dr. Leonie H. Mattison has worked inside public health, research, and human services when it mattered most: at the New York City Department of Health and Mental Hygiene after 9/11, at Sansum Diabetes Research Institute, at the County of Santa Barbara through the Thomas Fire and mudslide, and at CommUnify during COVID.

The space economy is young, but its organizational challenges are not: rapid growth, high-stakes missions, regulation, and the need to build a workforce that does not fully exist yet. Dr. Leonie H. Mattison has led through these pressures in public health, clinical research, government, and higher education, and has taught engineers at the community college level.

She is a Lean-certified executive and organizational development practitioner, and holds the ChatGPT Deployment Practitioner credential from OpenAI, bringing process discipline and technology fluency to every engagement.

THE THREAD is based on California’s Central Coast, home to Vandenberg Space Force Base, and brings experience building workforce systems alongside colleges, workforce programs, and community partners.

What is THE THREAD’s role?

Think of us as organizational architects and builders of human capability.

Your engineers may design the aircraft, vessel, spacecraft, technology, facility, or product. We help you design and develop the organization around it.

That means your structure, people, leadership, decisions, systems, culture, and capabilities: the organizational infrastructure required to move ambitious work from idea to execution.

Depending on what your mission requires, we may diagnose, design, teach, coach, build capability, integrate systems, or help lead the transformation alongside you.

You should expect a partner who understands your mission before prescribing a solution.

Your strategy, structure, talent, systems, leadership, and culture form a pattern. If one piece pulls against another, eventually the tension shows.

We help you see that pattern and redesign what is no longer holding.

Why is organizational transformation so difficult?

You may know exactly where you want to go. The harder question is whether the organization you have today can carry you there.

Your organization may have been designed for the mission, technology, workforce, or scale that came before. When your mission changes faster than your organization, you feel it everywhere: decisions slow down, talent gets stretched, accountability blurs, good people become frustrated, and strategy struggles to become execution.

A successful space mission doesn’t necessarily mean you are ready to scale. Your next constraint may not be the spacecraft. It may be a founder-dependent operating model, undeveloped leaders, unclear decisions, or an organization built for one mission rather than the next ten.

More demand doesn’t necessarily mean you need more people. If your roles overlap, handoffs are broken, or decisions sit in the wrong places, adding headcount can add coordination without adding capacity.

Digital transformation doesn’t necessarily mean you need another technology platform. Layering new technology over old workflows, unclear ownership, and inaccessible institutional knowledge can simply digitize your existing friction.

A new strategy doesn’t necessarily mean your organization is ready to deliver it. Your strategy can change overnight. Your structures, roles, incentives, capabilities, and operating systems rarely do.

Each practice moves in a direction. The Thread Movement Journal, Issue One, organizes the six practices across four movements: Inward, Backward, Outward, and Forward. What we mean by The Thread

The Thread is the underlying pattern woven through everything you have lived: the experiences, relationships, memories, and inherited wisdom that shape how you meet pressure and change. Pressure brings it to the surface. Once you can see it, you can choose what to weave next.

In an organization, team, or community, The Thread is the underlying pattern that holds together what appears fragmented: the history, decisions, relationships, routines, and inherited ways of working that shape how it responds to pressure. Once leaders can see it, they can reshape what the system keeps producing.

Pressure reveals patterns. Once a pattern can be seen, a different pathway can be chosen.

The T.H.R.E.A.D. System™ is six practices that help a person, team, organization, or community see that pattern, work with it, and redesign what comes next.

Where it began

It began on a grandmother’s verandah in Jamaica

Dr. Leonie H. Mattison’s grandmother was a quilter. On her verandah in Jamaica, she taught the lessons that became the foundation of The T.H.R.E.A.D. System™.

The six practices mirror the work of quilting

A quilt is not built from perfect pieces. It is formed from what exists: fragments, history, inheritance, necessity, creativity, and intention. The thread is what binds these parts into a coherent pattern.

Epistemology, autoethnography, and the discipline of practice

01EpistemologyQuilting as a way of knowing

“The language of thread, quilt, pattern, and weaving in this framework is not decorative. It is epistemological.” The framework draws on quilting traditions of women of the African diaspora and the Caribbean as carriers of memory, identity, artistry, survival, kinship, and cultural meaning across generations.

02AutoethnographyPractice examined with rigor

The framework was developed through autoethnographic practitioner inquiry: more than two decades of executive leadership across higher education, public systems, nonprofits, and workforce and community work, examined systematically and tested against established research.

03Patchwork ethnographyA guard against self-confirmation

A structured reflexive method that renders visible the seams of the inquiry, so the author’s interpretation is examined rather than assumed.

Our claim, stated plainly. Executive practice does not replace randomized study. The journal presents disciplined practice as a legitimate and distinctive form of evidence, and sets out a research agenda to test the framework further.

Pressure enters a person, team, organization, or community: a leadership transition, a funding shift, an enrollment decline, a new technology, a public moment.

Six practices across four movements

Interdependent and non-linear: enter at any practice, return as new patterns surface, or work several at once. Locate inward. Assess backward. Decide outward. Act forward.

Since Issue One was published, two movements have been named within these four: Downward, within the Backward movement, for Release Patterns, and Upward, within the Forward movement, for Adopt Change. Harvest Wisdom remains Backward, and Design Wholeness remains Forward.

What we mean

Coherence and wholeness are not the same thing

Fragmented

The system tells one story while operating from another.

It may proclaim trust while keeping decisions opaque, claim to be innovative while punishing risk, claim inclusion while concentrating authority, or claim transformation while preserving the incentives that maintain the old pattern.

Under pressure

Two ways a system responds

Conscious response

Names the issue, examines evidence, tests assumptions, and supports deliberate choice. It expands capacity for judgment, learning, collaboration, redesign, and implementation.

Unconscious protective response

Protects authority, belonging, competence, control, identity, legitimacy, or survival before the threat is fully conscious. In this sense, unconscious protection can appear as competence.

THE THREAD was built by an executive who has held the seat. Our founder brings presidential and CEO experience across higher education, government, public health, and human services, and our network of associate practitioners brings the depth each engagement requires.

A note from our founder

THE THREAD began with a question I could not set down: why do institutions devoted to transformation so often struggle to transform themselves?

For more than twenty years, I have led organizations through pressure. What I learned is that pressure is information. It reveals the pattern beneath the problem, and once a pattern can be seen, a different pathway can be chosen.

We built THE THREAD so leaders do not have to face that pressure alone. We bring executive experience, a proven method, and a network of practitioners who stay until the work holds. I believe leadership is a sacred act of stewardship. It is an honor to steward this work with you.

Our story

The Origin of THE THREAD™

Dr. Leonie H. Mattison has spent more than two decades leading institutions built to change lives, across higher education, government, public health, and human services, including as a college president and CEO. Yet the more responsibility she held, the more she confronted a difficult question: why were institutions devoted to transformation so often struggling to transform themselves?

She saw the answer from the inside, leading organizations through economic crisis, public health transformation, natural disaster, pandemic, and institutional turnaround. Different sectors, different pressures, same pattern. Many were still operating through systems, practices, and assumptions designed for a world that no longer exists. They were asking people to change, adapt, and grow while the institutions around them remained largely unchanged. Under pressure, leaders and organizations returned to familiar ways of thinking, deciding, and responding, even when those patterns no longer produced what they needed.

That insight led Dr. Mattison to develop The T.H.R.E.A.D. System™, a framework that equips leaders and institutions to recognize the patterns beneath their most persistent challenges and build the capacity to respond differently. Rooted in lessons from her grandmother in Jamaica, a quilter who taught her that every pattern carries a history and every thread holds tension, the system rests on a clear premise: pressure reveals patterns, and once a pattern can be seen, a different pathway can be chosen.

What began as a discipline forged through decades of leading institutions through complexity has grown into THE THREAD, a practice, movement, and learning ecosystem for intentional transformation. Today, Dr. Mattison and her team work with leaders, organizations, and communities to uncover the patterns beneath pressure, explore new pathways, and build the capacity to create the possibilities their futures demand.

  1. Chapter I1990s · Jamaica to New York City

    Crossing

    Globalization accelerates, immigration reshapes American cities, and the digital age begins.

    Between two worlds, an early lesson took shape: identity does not have to disappear in transition. It can be rewoven. The move from Jamaica to New York became a first threshold, and an encounter with the tension between what we inherit, what we leave behind, and what we choose to become. Becoming the first in the family to graduate from college turned that lesson into a lived pathway. It now informs FutureWoven™, designed to help young people imagine and build futures beyond what they can presently see.

  2. Chapter II2000s

    Leading Through Crisis

    Public health systems confront new pressures, the nation responds to September 11, and the 2008 financial crisis destabilizes communities and institutions.

    Across public health, economic development, and government, leadership meant entering systems under strain. The work included supporting public health transformation in New York City, including the transition of fiscal functions to a nonprofit partner, and navigating economic development through the financial crisis. Different sectors. Different crises. The same dynamic kept appearing: pressure did not create the pattern; it revealed what the system had been carrying all along.

  3. Chapter IIIMid-2000s

    Inquiry and Transformation

    Social media emerges, information accelerates, and the pace of change begins to outrun the institutions built to contain it.

    The questions being asked of organizations eventually turned inward. Lived experience became a site of inquiry, asking not only what needed to change, but what recurring patterns pressure was revealing. What emerged was not a management theory. It was six symbols, arising from a profound season of personal transformation. Each pointed toward a movement of the psyche, and eventually of systems: thinking, remembering, releasing, connecting, practicing, and creating. Those insights became the six practices of The T.H.R.E.A.D. System™, a framework later taught and applied across sectors.

  4. Chapter IV2011 · New York to California

    Threshold

    Scientific innovation accelerates while communities confront persistent gaps in health, employment, and economic mobility.

    A second crossing created another threshold. The Thread, LLC was established, giving the emerging work its first organizational container. In California, the work expanded through diabetes clinical research and then into organizational and workforce development for the County of Santa Barbara, including workforce systems for 4,600 employees across 23 departments after the Thomas Fire and mudslides. Another tension became visible: innovation could move quickly, while the human systems responsible for carrying it forward often could not. Transformation required more than new ideas. It required new capacity.

  5. Chapter V2018 to 2021

    Movement and Access

    Women challenge long-standing systems of power through #MeToo, Santa Barbara rebuilds after catastrophic fire and mudslides, and a global pandemic disrupts how communities live, work, and connect.

    The practices began moving from one life into many. The Thread Movement emerged in Santa Barbara County, followed by The Thread Collection, a faith-rooted body of work for women. More than 1,000 books carried the practices into correctional and community settings, and 72 women moved through W.A.I.T.™ as a structured journey of reflection and transformation. As executive responsibility expanded into human services through the pandemic, across more than 20 sites and 16 core programs, traditional systems of gathering, service delivery, and learning fractured. The work moved again. The Thread Institute began taking shape as a vehicle for extending transformation through learning, development, and wider access.

  6. Chapter VI2022 to 2025

    Leadership at Scale

    Higher education confronts post-pandemic disruption, demographic pressure, workforce upheaval, declining public confidence, and the arrival of generative AI.

    The work then faced its most consequential test: the presidency of a graduate institution navigating significant organizational change. Strategy, governance, accreditation, finance, enrollment, faculty systems, student success, technology, culture, and organizational design converged in one leadership environment. Working alongside the board, administration, faculty, and staff, the institution was turned around within two years, with a six-year WSCUC reaffirmation and five commendations, its first shared governance structure, fully online degrees, and the Pacifica Promise. The experience tested a larger question: could principles first discovered through personal transformation hold at the scale of an institution? They could. The work had become a way of understanding how people, organizations, and systems transform together.

  7. Chapter VII2026

    THE THREAD

    Artificial intelligence and accelerating social, economic, and workforce change are redefining how organizations work, learn, lead, and create value.

    What began as inquiry has become an ecosystem: one system expressed through three interconnected vehicles. The Thread, LLC brings organizational and talent design, development, and transformation to institutions. The Thread Movement, fiscally sponsored by the Santa Barbara Foundation, expands access through initiatives including W.A.I.T.™ and FutureWoven™. The Thread Institute builds the capability to practice, lead, and teach intentional transformation. With intellectual property being formalized, original curricula developed and copyrighted, more than 20 partnerships in development, and the Achieve Intentional Transformation podcast reaching thousands, the work is entering a new chapter. In 2026, The Thread, LLC was named an OpenAI Select Partner in the OpenAI Partner Network, extending the work into how people, systems, workflows, and AI work together.

    Why institutional coherence matters

    Transformational organizations turn pressure into possibility by strengthening the people, systems, and conditions that drive performance and impact.

    Pressure

    Pressure is rarely the problem itself. It is the force that reveals how an institution actually works: demand or enrollment shifts, funding tightens, a review approaches, a leader departs, a new technology arrives.

    Pattern

    Under pressure, institutions respond the way they have learned to respond. Those habits, decisions, and workarounds form a pattern that often stays invisible until pressure brings it to the surface.

    Built from inside the work

    The Index emerges from more than 20 years of executive leadership across higher education, government, public health, and nonprofit organizations, often during moments of crisis, and from sustained autoethnographic inquiry into how institutions respond under pressure.

    That work is documented in The T.H.R.E.A.D. System (2022) and in The Thread Movement Journal.

    Methodology

    How the Index is scored

    Each statement is rated on a five-point scale from strongly disagree to strongly agree. Dimension scores are combined into an overall coherence score and a dimension profile, and the lowest-scoring dimension points to the practice where work should begin.

    The free snapshot uses one statement per dimension, taken word for word from the full 18, so the two can be compared. It shows a direction; the full Index, available by invitation, gives a reliable dimension profile.

    The Index reflects the perspective of the person completing it. It is a starting point for conversation, not a full institutional audit. That is what the Coherence Diagnostic provides.

    Confidentiality

    Your responses stay yours

    Individual responses are stored securely and never shared. Responses may be used, anonymized and in aggregate, to advance research on institutional coherence. No individual or institution is ever identified, and no group smaller than five is ever reported.

    In a Coherence Diagnostic, individual responses are never shared with the employer; the institution receives only combined results.

    Human-designed, technology-supported. Secure technology organizes responses, applies The Thread’s interpretation logic, and surfaces patterns and personalized insights. Technology supports the analysis; it does not replace professional judgment. The assessment framework, interpretation logic, and recommendations are grounded in The Thread’s organizational and talent development methodology and established organizational practice.

    Building the evidence, one institution at a time

    Over time, aggregated results will inform an annual report on institutional coherence across mission-driven organizations, building the evidence for how coherence connects to pathways, possibility, and performance.

    Before you begin

    Who should take the Index?

    Presidents, CEOs, executives, cabinet members, directors, board members, trustees, and advisors in colleges and universities, government, public health, nonprofits, and other mission-driven organizations. It is especially useful before a strategic plan, a leadership transition, an accreditation or compliance review, or a major technology or AI investment.

    What is the difference between the snapshot and the full Index?

    The free snapshot is six statements, one per dimension, shown one at a time, followed by a few short questions about what you are navigating and what you want. It gives you your coherence score, the pattern beneath it, what it may be producing, a pathway worth exploring, and three things you can begin today. The full Index adds twelve statements for your complete dimension profile, a 90-day pathway, and the full executive report. It is available by invitation: after your debrief, Dr. Lee can send you a private link or access code, and your snapshot answers carry over.

    Is it really free?

    Yes. The snapshot has no cost and no obligation. If you would like to discuss your results, you can book a 30-minute debrief.

    Is this an audit or an accreditation evaluation?

    No. It reflects one respondent’s perspective and is intended for reflection and planning. For an institution-wide view, apply for a Coherence Diagnostic.

    Can I take it more than once?

    Yes. Retake the Index over time to compare how your perception of institutional coherence is shifting and identify questions worth examining more deeply. Because the Index reflects one person’s perception, stronger conclusions about institutional change need many perspectives, which is what the Coherence Diagnostic provides. To keep results meaningful, each email can complete the Index once every 24 hours, and each attempt is saved separately.

    What happens to my information?

    Your responses are stored securely and used to prepare your report and respond to you. With your consent, they may be combined with others, with names, emails, and organizations removed, for research and benchmarks. No individual or institution is ever identified. See our privacy notice.

    How we handle your information

    This notice explains what information THE THREAD collects through this website and how it is used.

    What we collect

    When you submit a form or complete the Leadership Self-Reflection or the Institutional Coherence Index, we collect only what you provide: your name, email, title, organization, country, and your answers. Nothing more.

    The THREAD Check-In™

    Your Check-In responses are processed in your browser to create your reflection. They are not sent to us or stored on our servers, and no AI is used to create your reflection. If you choose “Keep on this device,” a short summary is stored in your browser’s local storage on that device only, so a future Check-In can ask what has changed; you can remove it at any time with “Forget my saved Check-Ins” or by clearing your browser data. If you choose to email, share, or print your reflection, it goes only where you send it. If you contact us or subscribe, only the information you enter in that form is sent.

    Why we collect it

    To create and send your report, to respond to your inquiry or follow up if you ask us to, and, in anonymized form, to advance research on institutional coherence. If you opt in, we also share occasional insights. We never sell or share your information.

    Where it is stored

    Securely in Google Workspace tools, accessible only to Dr. Leonie H. Mattison and associates who have signed a confidentiality agreement. A notification of each submission is sent to our team, and assessment reports are emailed to the address you provide.

    How research use works

    With your consent, responses may be combined with others, with names, emails, and organizations removed, to study patterns across institutions. Research is done on a separate, de-identified copy of the data. No individual or institution is ever identified in anything we publish, and we never report results for groups smaller than five.

    If your institution engages us

    In a Coherence Diagnostic, individual responses are never shared with your employer. The institution receives only combined results, and no group smaller than five is reported separately.

    How long we keep it

    Identifiable information is deleted or anonymized after three years.

    Retakes

    Retake the Index over time to compare how your perception of institutional coherence is shifting and identify questions worth examining more deeply. You can take it once every 24 hours. Each attempt is saved separately. If you complete the full Index after your snapshot, it is added to the same record. The full Index is available by invitation, through a private link or access code.

    How we protect submissions

    Our forms use Google reCAPTCHA and automated checks to block spam and misuse, and we limit how many report emails any address can receive each day. Use of reCAPTCHA is subject to Google’s Privacy Policy and Terms of Service.

    Your choices

    You can request a copy of your information, unsubscribe, or ask us to delete it at any time by emailing leoniemattison@thethreadllc.com.
